Other Means of Referral
If you prefer or deem it more appropriate, feel free to provide our contact information to your patient, and when they reach out, we will get them scheduled, obtain a release of information to consult with you directly, and follow up throughout the assessment process. We will still need a referral form completed in order to bill insurance.
Once your client/patient is scheduled, we will send you an Appointment Confirmation form with the date they are scheduled.
When the appointment gets closer, you will be scheduled for a consultation with the Psychologist providing the assessment.
